This is a reasonable circuit, though there is not enough information to assess it.
1. Why are both C98 and C99 present?
2. If the fuse is 40Ohms, you will see a large drop at pin 2 during startup, as the output current will be at short circuit (around 27mA) until C98/99 charge up to 5V.
3. Is there a DC load on Vout? Where does REFIN go?
1) We followed the Eval schematic of AD4134 for our project. I am attaching the pdf of AD4134 Eval Schematic. This reference IC circuit is present on Page 3 of the attached pdf. It contains those capacitors
2) We have noted your point, and will replace with the suitable fuse that will result in lower voltage drop.
3) Vout (REFIN) is connected to REFIN pin of AD4134 IC.
