ADV7391 Power Supplies

ADV7391 datasheet, please take a look at page 72/108 and tell me what does
AGND, PGND, DGND and GND_IO means? Should I use four different power supplies?
How should I connect these four grounds together and where. The information on
the datasheets is misleading.

The 4 different ground types are related to internal sections of the chip.  Normally we tie all the grounds together on a single plane.  Check out the reference layout.

Vdd_io and Vaa are nominally 3.3V rails while Vdd and PVdd are 1.8V rails.  These sources can be generated by 2 different regulators.  Figure 94 has ferrite beads separating the various power pins to isolate AC coupling between the internal sections of the chip.  You can use 4 LDOs or regulators but 2 will suffice as long as you have the ferrite isolation.
