AD8233 Eva-board  +Vs input bias was no-signal

AD8233 Eva-board set +Vs bias mode and connected test circuits. Next 10mVpp signal input +IN.

 Its output was observed a normal signal. 

But the input signal added ±300mV offset , it didn't output. Why ?

Other input bias REFOUT and RLD was no ploblem.

Eva-board setting was below

FR :Low 

AD/DC :Low (DC Mode)

SDN :High

RLD SDN :Depend on bias setting

Jumper was default with out bias pin.
