i used AD8220 in simple with gain=1(no resistor).
i expect Vo swing between +/-5 Volt according to Vo = G • ( VIN(+) – VIN(-) ).
i have large offset at output.
what is wrong in my circuit?

I would think that you may be running into diamond plot limit. To confirm this, can you give me the exact voltage that you applied on the inputs and its corresponding output voltage that you measured.
Also where is the connection of the REF pin? Make sure that it is not left floating.

inputs are between 0 to 5 volt and i expect output between -5 to +5 volt.
I can see your schematic. However I need to quantify how large is the "large offset" that you are seeing.
Can you share to me any data that you collected (Vin vs Vout)?
Can you also confirm where is the REF pin connected?
What is the source of your input signal? Can you make sure that it has common ground with the supply voltage?
