AD8361 - Possible Reasons for Failure

We have been using the AD8361 ICs for quite a long time, in both receiver and transmitter applications with various waveforms and varied input power levels. Throughout all this time we have very random failures (i.e. 1 out of 100 or more) where the detector will stop reporting an output. We normally have chucked that to mishandling of the part (maybe ESD damage, maybe thermal stress due to rework operation) since the occurrence is so sporadic. We've recently been asked to look again into this, so I am coming here to see could we expect to be common failure mechanisms for this IC, and how would they manifest themselves?
