How to import  properly AD4528 SPICE model?

I want to simulate the behavior of the amplifier with specific feedback in SPICE.

I loaded the .cir file downloaded from the product webpage. Then right-clicked on .subckt and "create symbol".

SPICE auto-generated symbol for the amplifier, however when I click on simulate I get the following errors:

This circuit has floating nodes.

 

what am I doing wrong?

Can I use the default opamp2 schematic symbol even though the node assignment of the symbol and the .subckt are different? (1,2,3,4,5) and (1,2,99,50,45)
