Medical Imaging: 1080p30 input SDI/HDMI/DVI minimum latency to output multiple formats

We are designing a device that has switchable video inputs (SDI 3G, HDMI, DVI, S-Video) up to 1080p30 and require the lowest latency to the video outputs that can be several and different formats all at the same time (SDI 3G, HDMI, DVI, Display port, S-video 4:3).  For example the outputs need to be switched on/off (software controlled by our CPU) and may be in 16:9 format and/or 4:3 (for older style monitors).  In all cases the outputs may require different (scaled) outputs to match the displaying monitors and more than one monitor may be connected and viewed.

 

So, we are seeking the best 2 or 3 package solution to manage these high bit-rate signals with the lowest possible latency - as the use is for medical endoscopic input and display as close to real-time as possible.
