"Failed to set breakpoint" error

I have a Visual DSP++ 5.0.8 project group set up with 3 projects.  One of the projects contains 6 header files and 6 source files.  One of the source files, xyz.cpp is quite stubborn.  It seems to produce no debug output or something strange is happening.  All other files in the project allow a break point to be set normally (i.e. only when the execution is halted).  But this one file refuses to let me set a breakpoint and the output window reports "Failed to set breakpoint in "xyz.cpp" on line 82" when I try.

Another thing I noticed about this deviant file is that I cannot right click in the file when it is opened and select "Mixed" to view the disassembly like I can with all other files in that project

I have verified that this one deviant has its File Options configured to use the project-wide settings.  The project settings are configured to produce debug output.  What else could be causing this file to be cranky?
