Sounds like you need an electrometer. Go to the Keithley website and order their free book on low
current measurements. Talks about how to build curcuits and how to eliminate small errors.
at these current levels you will need an buffer (instrumentation amp) that have very little input current (bias and or leakage). Non of the products labeled "Power Monitors" will be able to achieve accuracies at these current levels.
Please explore the Instrumentation Amplifiers section to find something suitable for your common mode voltage range.

A more specific answer will depend on many factors such as common-mode voltage range, voltage rails, temperature range, measurement speed, etc. As a starting point, you could look at AD8220 or AD8422. Please let us know if any of these would work of if you need further help.
