Does anyone know what the DC voltages on each pin of a correctly working AD8302 running off 5V would be?
The DC voltages on the inputs should be about 100 mV below the positive supply rail, so for you it would be about 4.9 V.
If you are talking about the random DC offsets, those are nulled out with a DC offset correction loop.

Another question, would it damage the IC if the input signal was 2Vpp square wave (about 20dBm). I am only interested in the phase output so if no damage occurs will I still get accurate phase results?
From my understanding of the input circuit my input signal will be clipped by the diodes. And this clipping shouldn't affect my signal. Correct?
You will probably damage the part if you put 20 dBm on the input; max level is 10 dBm. Your signal will be clipped, but you could still damage the input stage with a 20 dBm signal. I would advise attenuating your signal before applying it to the AD8302 input.
