Noise contribution from an amplifier

Hi

 

I am a little confused on how I should measure the total noise contribution from various noise sources when I am using a differential amplifier. The IC that I am using is AD 8009 and I do not know which noise sources to consider (from out of thermal(johnson), input current noise and input voltage noise) and how they should be added to give the final noise in the output, say in terms of the final output voltage.
