AD8218 Blowing up at 60V

Dear All,

PFA the schematic used to monitor High Side telecom rectifier currents. The circuit when powered up at ~ 60V (w/o shunt, i.e pin 1 connected to +48V and pin 2 open) blows up the chip and the track circled gets fried.


I have also tried adding a 4.7K resistor in the place of C10 in case it is because of a differential created between the input pins but no luck.
