LT3758 SEPIC startup failure

I have a SEPIC design using the LT3758 out of about 30 built 3 have failed damaging the inductor and or the output diode. This failure appears only to occur on startup and I have been unable to duplicate the failure on the bench. using an electronic load the maximum current the board will deliver is 1.8A. The inductor is rated at 3A RMS and the diode is also 100V 3A diode. My problem is that I don't understand how the circuit could deliver enough current to damage the inductor or the diode?
