AD8421,  there's a 10V voltage between +IN and Vout

The circuit like above,the source is ±12V.  When the power on and no input, there's a 10V voltage between +IN and Vout.  when input a signal, the voltage disappear. and its input impedance is not as large as the datasheet. what's the reason?  I bought 25 chips, they are all same.
