why do want to replace the LT8640? It's a fairly new part with very good performance and offers the SilentSwitcher features which should ease your design drastically in terms of EMI approvals.
The LT8640S with a slightly larger package offers SilentSwitcher 2nd Generation features (integrated capacitors) and 1A more rated output current than LT8640.
The mentioned ADP1621 as Boost controller is nothing similar to an integrated Buckregulator like the LT8640.
I could not find it anymore to purchase so I replaced it with LT8643, which comes from the same family. Is there any significant difference between them related to schematic config or layout?
This is about a medical device i use it to, bassically step down from 12V to 5V, but I am interested in good EMI filtering.
the LT8643S is already a 2nd generation part of our silent switcher family, with its integrated caps inside to reduce the size of the hot loop even a better part than the LT8640. And it offers 1A more output current. But both parts are new and should be available easily. LT8643S has a bigger package. Comparison of both is here (Together with the LT8640S):

I assume for a medical application the part should be fixed for your approval process, correct?
