    The LTC1564 is a LPF digitally programable frequency 10kHz to 150kHz in 10kHz steps and gain 1-16 is 1V/V step.

    For higher gain use a fixed gain (5V/V to 10V/V) difference amp, diff to single input stage with a low noise op amp, LT6202.
