how to make a low phase noise amplifier with a chip

I don't have expertise in design of low phase noise amplifiers. Even if I did, the discussion would probably be too broad for this EZ forum. However, using an amp such as the HMC606 which is fabricated on a low phase noise process technology such as GaAs InGaP HBT, is a good start. The easiest way to achieve your final goal of having a low phase noise amp in a box with two (RF connector?) ports would be to choose the packaged HMC606LC5 rather than the HMC606 die...

...then purchase the corresponding EVB and use that:
