Hi,
1) 3.3 Vpp is the maximum rating for reference input as given in electrical specifications. Table 2 gives the suggested values and the important point is to provide 0.5 V/ns slew rate without exceeding 3.3 Vpp.
2) Output is single ended and is connected to pin 28.

VOL = 10% Vdd and VOH = 90% Vdd which corresponds to approximately 0.33 V and 2.97V.
