EVAL-HMC1023LP5 and HMC6356 USB Interface

I have a EVAL-HMC1023LP5 and driving it with a HMC6356 USB interface card. This interface card appears slightly different them the one shown in the the photos 

The usb interface card is using DLP2232H. https://www.dlpdesign.com/dlp2232h-ds-v11.pdf

There does not appear to be a SPI output coming from this output header. Do you have a schematic for the HMC6356?

    Jumpers on DLP2232H (HMC6356) module should be set as JP1 short, JP2 and JP3.

    There may or may not be a solder bridge on the underside of the interface board's header. If there is a solder bridge then there is an important case with the bottom solder on pins of eval board and jumper states which must be definitely avoided. If pin2 and pin3 of JP2 are shorted while the bottom solder is there, DLP2232H breaks. This configuration directs 5V to the GPIO supply of DLP2232H module which exceeds its absolute maximum rating of 3.3V.

    Could your interface have been damaged in this way?
