It depends on the part, and temperature. Down at the low end of input current range, there will be more temperature variation:

If mass production application, see also datasheet Figures 11 and 13, and compare that against your accuracy requirement.

In Figure 1, the arrows point to copper that connects to the VSMx pins.
In Figure 2, we recommend a defined guard ring around the INUM node connections. Ref. datasheet page 26.

You're correct. When VSMx= Gnd, the guard ring can also be Ground.
