Could you provide more details of your frequency, supply/digital voltages and what RFs are turning on? For certain voltage applied to A/B/C, what insertion loss and isolations do you measure on the other channels?
Has this problem just started occurring? Or is this a time = 0 condition? I would also check to make sure that the part is still functional and not sustained damage, and the assembly to make sure that there aren't shorts/opens.
