Frequency Divider issue

We have been using the frequency dividers HMC365, HMC363 and HMC 365 (divide by 4, divide by 8 and divide by 4 in sequence to get 60 MHz from 7.68GHz. Enough care is taken while assembling these dies (ESD and other safety procedures followed) and also proper dc blocking capacitors are used in between the ICs. Single ended configuration is followed for all the ICs. We are very often facing the problem of failure of one divider among three dividers in this cascade approach. More often the divide by 8 IC HMC 363 placed in between divide by 4 is getting failed. Can I get any explanation for this failure?

Can these cascade approach be used? What precautions need to take to safe guard the ICs?
