Can LTM4668A be configured as two single outputs (ch1 and ch2) with ch3+ch4 in parallel for a TRIPLE output configuration?

Yes, it can be configured in this manner. 
 Please note that channel 4 is the master channel when paralleled with channel 3 and also that PGOOD4 is the relevant indicator that the output voltage has been achieved.
