Why is the efficiency of LTM4705 in LTSPICE and LTPowerCAD different?

The efficiency results can differ between LTSpice and LTPowerCAD for a number of reasons but very often it is related to ideal component models being used. 
 For the efficiency report below from LTSpice, the problem is that the capacitors ESR was not accounted for in the simulation, as their power dissipation cannot realistically be 0 mW as shown below. 
 To align the results with LTPowerCAD, the ESR must be included in the model.
