Relation between input voltage, output voltage and bias supply

Dear Power Management,

we have some questions on the device LT3073:

- There is stated in the datasheet, that the bias supply has to be 1.2V above the output voltage. Is there any dangerous condition, if the bias supply is present and the input voltage is not?

- Is there a recommended power up sequence related to bias and input voltage? The user guide implies (1. input, 2. bias), but no clear statement.

It seems we crashed one device, but it also could be a reverse current issue.
