We are using ADP2164ACPZ-R7 for +5V to +2.5V step down conversion. As per datasheet we designed the hardware and tested in few boards(15 No's) in one lot it is working fine. Later we remodified the hardware
not related to this portion and reloaded the board and ADP2164ACPZ-R7 Ic's with new lot assembled the board. Out of 11 boards 3 are working fine all other boards we are getting +5V in place of +2.5V.The input and
output getting shorted to GND. So we are unable to move further, high current it is drawing. As per application circuit Figure 44 on page 18 of 20 we are using to generate output voltage +2.5V as below

can any one suggest to resolve the issue what could be the problem as soon as possible .
The new lot IC's on TOP we are able to see 2164,ADJ,#2319 and 76861. We are getting more failures with this lot.
