Looks like the output is being pulled positive for some reason. I have seen when the output is connected to the negative of an op amp the positive voltage passes thru the op amp and pulls the output voltage positive. With no load it should not be positive. Is it possible to tell if the EPAD is soldered well? This serves as the GND for the IC. The only other suggestion is to check the solder connections. The only thing missing on the schematic is the input capacitor. That should be placed between VIN and GND and close to the part. Below is a recommended layout from the datasheet.
