We are using the 750312504 for a project with LTC3300-1.
On your calculations for your EVB you are calculating the RTONS/RTONP assuming that the primary inductance is 12uH but the transformer's datasheet says 3.6uH.
Is this a typo or something that I missed out ?

The document is actually for 1A design and the DC2100 is a 4A design. The calculations can be used for your application but you would need to use the values required for your application.
I believe the transformer used in this calculation is from the attached file.
