We are baffled and wonder if you can solve this for us.
Our supply labeled +15V is +12V.
Pin 9: 1.592V, pin 11: 4.25V, pin 6: 2.8V, pin 7: 3.93V.
We are expecting -12V on pin 1 but getting 0V. The results are the same both with and without load.

From your schematic, it seems that you connected the SYNC/FREQ pin of the ADP5074 to 3.6V, since I assume that D8 is a zener diode. However, it can also be connected to VREG if you plan on using the 2.4MHz switching frequency of the ADP5074.
Have you tried looking at the SW pin (pin 1) waveform of the ADP5074?

The voltage on EN was increased to 4.2 so that SYNC was in a high state, but this did not change anything.

1. Please review 150uF cap at output and 100K soft-start resistor plus output current. Make sure current-limit is not violated during start-up and steady-state. You may use this as reference: www.analog.com/.../preventing-start-up-issues-due-to-output-inrush-in-switching-converters.html
2. If you can measure VIN, VOUT, VSW, IL on oscilloscope and post so I can review, please. Both at start-up and steady-state, zoomed-out and zoomed-in.
