Why is my Voltage Regulator circuit (HMC1060) producing weird voltages?

Hi all, I am using the HMC1060LP5E with an input of 5.5V to provide four voltages out. VR1= 3.3V, VR2=5V, VR3=3.3V, and VR4=5V. The loads on all of these outputs only draw a few mA of current. The problem is that I'm getting VR1=2.69V, VR2=5.5V, VR3=2.69V, and VR4=4.1V. I'll attach the schematic for the circuit. Any suggestions would be greatly appreciated. Thank you!
