LTC3859 - pcb reference design

Question

Hi, 
 Could you share with pcb reference design please? 
 As I do not found eval board for this IC I design pcb. Unfortunately looks like I did something wrong because load on Vout2 makes channel Vout3 blow up (if also under load). 
 So only Vout1 and Vout2 or Vout1 and Vout3 can work in same time. 
 I used your circuit Figure 15. High Efficiency 1.2V/3.3V Step-Down Converter with 10.5V SEPIC Converter.

 From looking at Figure 15, all three DC-DC converters are independent from one another and all they share is VIN. VOUT3 stability should NOT depend on VOUT2 operation. I would check the layout to make sure switching nodes and gate drive traces are NOT routed close to or underneath sensitive nodes such as ITH, FB, SS, FREQ, and MODE pins.)
