ADRV1CRR-FMC: SOM Powering Failure

Hi I'm using ADRV9361-Z7035 SOM board on ADRV1CRR-FMC Carrier board. The carrier was working properly for 2 weeks after that every time, I switch on the power of the carrier boardI got the following behavior according to SD card location

SD card (FMC carrier)

The Red LED, DS8 of the Carrier illuminates, while the on the SOM board the Power Good LED D3 is off and LED D4 forever green. Without any configuration being applied to the board as DS7 is off.

SD card (SOM board)

The Red LED, DS8 of the Carrier illuminates, while the on the SOM board the Power Good LED D3 is off and LED D4 illuminates for 5 seconds then turns off. Without any configuration being applied to the board as DS7 is off.

I've searched for such issue and I see that this may come due to power sequencing done by the PG_MODULE signal as it can be pulled low by the carrier board power ICs, the FMC board, or the PicoZed when the board’s power circuitry is not Good yet. I've tested the Picozed SDR "ADRV9361-Z7035" on another FMC carrier board and it works and configured properly with (DS9 --> Green, DS7 --> Blue, DS8 Red) on the FMC carrier and (D4 Turns green for few seconds then off and D3 always Green).

So, the issue looks like to come from the newly arrived FMC carrier board. I've tried to use the adapter of the working FMC carrier, to reset either the FMC carrier or the SOM board itself but nothing works. I've also looked deeply into the carrier board for any physical damage, but everything is well without any damages.

Note

The power supply correctly provides 12 V to the board, with "SUPPLY TEST" (P21) debug points always at 12 V. I also measure 5 V on E11(Carrier) and on one side of C301(SOM Board) when the board is switched on. Here are also some other measurements on test points and an P23 are tabulated:

Moreover, I was unable to connect serially to the carrier via USB-UART port "DS2 is off" to try re-flashing the power sequencer. Please provide me with help how to factory reset the FMC carrier or with any solution to such issue, as now I've only one FMC carrier working.
