Part not getting programmed.

It may be a power up sequence issue. In the datasheet, there's a 300us required after /RESET high to reset the X, M, and C registers to their default state. During this time, you cannot write. 



You can try powering up the analog supplies (VDD and VSS), then logic supply (DVCC), then /RESET. And wait before writing data.

Attaching a better image. Please note that the RESETb and CLRb pins are tied to DVCC. Could there be a race condition happening if DVCC(3.3V), RESETb, and CLRb come up together but maybe slightly before the VCC/VSS signals?

Have you tried using the IC not getting programmed swapped to a working board? Just to isolate the issue.
Usually, the datasheet indicates the recommended power sequence. In case it didn't, it's OK to follow this sequence:

1.  Ground (digital and analog)
2.  Digital supply
3.  Control digital pins (e.g. RSTSEL)
4.  Analog supplies (AVdd and AVss)
5.  Interface lines
