Please provide the thermal resistance junction to case for this device and also
the maximum operating junction temperature.

The AD664 is quite an old product for which is difficult to obtain any sort of
information.
As I could not find anything I contacted our Assembly department.
Please, find their inputs below. I hope it helps.
The AD644SD-BIP is a 28 lead SBDIP.
The only thing I can see that comes close is the CERDIP in ADI0121:
