Hi,
The input common mode refers to the red circle as shown in the picture above. The resistor RG1 & RG2 are internal to the part.
Yes ADL5565 can be configured for attenuation mode such as -6dB. Refer to Page Pg 19 / Fig 36 / Table 10 on the Datasheet.
