Problem in AD8219 Spice macro model

Have you tried using a current source to simulate the input current?

Hello krisf, Thanks for your reply. No actually I couldn't try an input current source, because in my system the input is a DC voltage source decoupled by several large electrolytic capacitors. Please have a look on my attached schematic. Notice that I modeled the load of my circuit as a resistance of 5Ohms, while in my actual application the load is a full-bridge LLC converter. The resistance here represent the average current would be supplied by the DC source.

Currently, the AD8219 give an output of 630 mV for a differential input of 7.2 mV, which is around a gain of 87!

For the purposes of simulation, could you try shorting Vs to the 36V voltage source? I tried it on the simpler schematic (the first one that you posted), and I was able to get the output to behave correctly. 

Let me know if this works for you as a temporary fix.
