LTC6090 HV Half bridge design

Hello,

I've designed a half-bridge ''high-voltage''... kind of work, but:

1. Fairly stable but limited BW, I need 1Mhz -3db, I get about 300KHz ?

2. The op-amp has severe latch-up problems ! is it known the LTC6090 latch-up ?

3. With a supply -12V/+110V, what is the max. voltage input of the LTC6090 (common / differential) ?

4. With the frequency increase, from about 50KHz, the output offset varies a lot, not sure what will be the cause (practical or theoretical) ?

    It is sorted, I am using a resistive voltage divider, and the Vin+ current intake (which varie whit frequency ?) affects the Vdrop across the resistors, I've used, for now, much lower resistor values for the voltage divider.

    The rest still not as it should be...at least on paper...?
