I have downloaded the SPICE model of AD8397 from the following link that comes in *.cir and not in *.lib format.

When I opened the model file, I found some strange pin numbers. How they can be aligned to the pin numbers in datasheet ?

As can be seen on the .cir file:
1 - corresponds to the +IN input pin
2 - corresponds to the -IN input pin
99 - to the +Vs pin
50 - to the -Vs pin
4 - to the OUT pin
The current AD8397's spice model which can be downloaded from its product page can only be used for single channel / single op amp operation. To fully replicate the operation of AD8397 which is a dual channel / dual op amp, you'll need to use two AD8397's on LTSpice for dual channel operation.
Another concern about this model is that currently, it has some issues that are being fixed. A new AD8397 spice model will soon be released on LTSpice after it has been verified. Can you wait for this new model? I apologize if the old model on the web has some issues.

The new AD8397 model is already available on the LTSpice Components Library: 
To be able to see the new model on the library, please update the library by clicking Update Components under Tools from the Home Menu.
Please make sure also that you are using the latest LTSpice version (v.24) which can be downloaded via this link: LTspice Information Center | Analog Devices
