LT1357 output is very bad

Question

I use the LT1357cn8 to amplify a sine wave input from 0.6V to 2.4V. I built a close loop circuit with a gain of 4 and the input has a maximum frequency of 1.5MHz. When I measure the output with an oscilloscope the sine wave becomes distorted at around 10kHz(see the following pictures): what is causing this?

Hi forzinkahd: The LT1357 needs headroom from the supplies. How close is the input or output to ground, for example? The inputs and output want to be about 2V away from either positive supply or ground.)
