I followed the datasheet to the spec for designing the G+2 op-amp using AD8004. But I am not getting expected results. My input signal is a 1Vp-p and what I am seeing at the output is ~520mV. I adjusted P1 so that P1+R92=R93. The input through R94 is for offset adjustment only. Any ideas why my output is not 2Vp-p?
From datasheet it says for G+2, Rf=Rg=604ohms.

How are you measuring your signals?
Are you using high impedance probes right at pins 1 & 3 of the IC or are you just assuming that you are feeding 1V (because that's what the generator is set to) and run this into a 50 Ohm terminated scope via your output connector?
Make sure that the P1 R92 connection is not open as this would turn the amplifier into a follower.
This is a very simple circuit that should be easy to debug with a scope...
