EVAL-AD8494 power supply

Yes the EVAL-AD8494 can work at single 5V supply. Just connect the GND and -VS header pin in P_SUPPLY together for a single supply configuration. However, if you want to measure a negative temperature in a single supply configuration, a positive voltage less than +VS should be applied to the reference pin to shift the output. Remove the jumper from J2 so that the reference pin is not grounded.
