Simulation problem- LTspice model of ADA4930-1

Question

Hi, 
 I need single to differential Amp, low noise and abut 1MHz to 200MHz, G=1. 
 I found this amplifier, I performed a simulation in the Diff-Amp Calculator software and it looks good, but the LTspice simulation I don't see that the model works for me accordingly. 
 Diff-Amp Calculator: 
 
 LTspice: The G is not 1, the amplifier fails me for some reason.

Hi Barak, 
 The ADA4930-1 is only operational up to 5V supply. The Diff Amp calculator software also gives this warning on your configuration (you configured it to +/-5V or 10V single-supply equivalent). 
 I attached a sample circuit with a gain of 1 based on your configuration. I set the Vocm to 2.5V though. please see and let us know if you have more questions.
