Pinout for Supply Voltage

Question

The dc supply pinouts VDD1 and VDD2 for the HMC451 amplifier I want to make sure are the left and right most pins connected to 5v supply and the middle pin to GND?

 You are correct. Based on the evaluation board layout, J3 and J5 are DC pins, while J4 serves as the ground (GND) connection.
